Assessment and Application
For Long Term Care and Supportive Housing
Any resident of Manitoba, of any age, registered with Manitoba Health who:
-
requires health services or assistance with activities of daily living
-
is a caregiver that is requiring support to meet an individuals needs
-
requires more assistance than they are currently receiving to remain living safely
at home
Anyone may refer an individual to Home Care for assessment by contacting the new referral
line: (204) 578-2339.
(a) Application and Assessment for Personal Care Home may be
initiated when care can no longer be managed safely in a person's own home even with the support of
family, home care and other resources. The Home Care Case Coordinator discusses with clients' and
their families the eligibility for admission into a personal care home and completes the
assessment. The application includes consent, choice of facilities, a medical, nursing and social
assessment.
Prior to making the final choice clients will be encouraged to visit some of the
personal care homes.
Once completed the Application for Personal Care Home is forwarded to a Regional
Panel for review. Upon acceptance the clients name will be placed on the waiting list and moved up
accordingly. If the client or their family has any questions or concerns about the application or
the 'paneling' process don't hesitate to call the assigned Case Coordinator.
(b) Placement process
There is generally a waiting period before someone is admitted to a personal care home.
During this time the goal is to support a clients' care safely at home. If someone is waiting
placement in the hospital or requires more immediate placement into a personal care home, they may
be asked to accept placement in a different home than the first choice indicated. This will be to
meet the immediate care needs. It does not slow down the movement to the personal care home of your
choice. A clients name continues to move up on the waiting list.
If there is a disagreement with the decisions made regarding eligibility or level of care
and after discussing them with the Case Coordinator and Regional Management an appeal to the Appeal
Board for Home Care can be made.
