Home Care Programs
Assessment and Case Management
Anyone may refer an individual to Home Care for assessment by contacting the new referral line: (204) 578-2339
When a referral to Home Care is received a Home Care Case Coordinator arranges to make a visit with the client and their family Together, they will determine the eligibility for service and conduct a complete assessment of the client's needs. The Case Coordinator works with the individual and their family to develop a plan of care that organizes assistance for the clients' needs. These services may be through home care services as well as assistance in providing information and access to other community resources.
Your Case Coordinator will continue to work together with the client and their family to evaluate the plan of care on a regular basis. If you are in a hospital and need home care for the first time, a hospital based Home Care Coordinator will assist you with your planning for discharge home.

Home Support
Home Support Workers assist with activities such as meals, minimal housekeeping and laundry to support a safe environment.

Personal Care Assistance
Home Care Attendants provide assistance with activities of daily living that may be required. This could include help with bathing, dressing, and toileting.

Therapy Services
Therapists may visit a client at home to help with exercise programs, alternative ways of managing activities, and assessments for safety and equipment in the home.

Nursing Services
Home Care Nurses provide health teaching, counselling and care in the home. Often they provide care such as I.V. therapy or treatments, in the home when the client is being discharged from hospital or to avoid going to hospital.

Equipment and Supplies
Some equipment and/or supplies may be available through the Home Care Program.

| Self and Family Managed Care Option
Self-Managed Care is a Home Care program option under which funding equivalent to the cost of Home Care non professional services is provided to assist eligible Home Care clients in taking full responsibility for arranging, coordinating and managing those non-professional services required tomeet their assessed home care needs.

Family Managed Care Option
Family Managed Care is a Home Care program option under which funding equivalent to the cost of Home Care non professional services is provided to assist eligible family members of Home Care clients in taking full responsibility for arranging, coordinating and managing those non-professional services required to meet their family members assessed home care needs.

Home Oxygen Concentrator Program
Provision of concentrator delivered oxygen for individuals medically meeting the criteria , environmental safety and competency eligibility who require continuous long term oxygen to live independently in the community

Respite Care
Relief for the care provider may be arranged for longer periods of time. This may occur in home or a personal care home.

Family Relief
Short periods of in home relief for care providers may be arranged to allow them time for activities outside the home.

Adult Day Program
The ADP program is for older adults who are unable to go out on their own to participate in programs and activities. The program is designed to improve quality of life by maintaining or improving functional ability and providing opportunity for fun, fitness and friendship. In Brandon, Fairview Home operates the ADP five days a week. |
Supportive Housing
Supportive Housing is a community residential setting that provides 24-hour support and assistance for frail or cognitively impaired elderly individuals.

Palliative Care
The Home Care Program is part of the Brandon Palliative Care Service this includes the inpatient unit at the hospital, the Home Care program, the Canadian Cancer Society and WATCH (Westman Association for Terminal Care and Hospice).
Together these services strive to:
- provide comfort through pain and symptom control.
- assist the client in meeting their physical needs.
- provide emotional and spiritual support to the client and their family during a very difficult time.
- support the client and their family to enable them to remain at home, if desired.
