The Home Care Program provides: Support to help people stay at home for as long as possible. When staying at home is no longer a safe choice, Home Care will help plan for future care.

Who Can Receive Home Care? To be eligible for an assessment visit by a Home Care Coordinator, you must:

live in Manitoba and have a Manitoba Health Care number AND
need health services or support with your day-to-day activities OR
have a caregiver who needs help to care for you OR
need more help than you are receiving to remain living safely at home.

How Can You Contact Home Care? Anyone can call Home Care for an assessment.

Call the Referral Line at 578-2339. This phone is a voice mail message system that is checked several times a day. Simply leave your name and phone number and someone will call you back.
If you are a patient in the Brandon hospital, you can ask your nurse to contact Home Care for you.
If you currently receive Home Care and have any questions, call your Case Coordinator. If you are unsure of the number, simply call the Home Care office at 578-2325 and they will help you.

What Happens When You Call Home Care? A Home Care Case Coordinator will call you and plan a visit with you at home or in the hospital. The Case Coordinator is a nurse who will complete an assessment with you to identify:

how you are managing
what help you may need
what kinds of supports can be provided by others
what supports you may receive through Home Care
if a personal care home is needed

The Case Coordinator will help you make a plan to meet your needs.
